Personal Medication Record

You will receive a personal medication record (PMR) at the conclusion of your MTM consultation. Your PMR will list:

• Names of all the medications you take
• Instructions for how to use your medications
• Names of the prescribing doctors
• Vitamins
• Important tips to help you to get the most from your medications

The PMR is a great tool for you and all of your health care providers. The PMR will help keep all of your health care providers informed of any changes made to your medications. Bring your PMR to each visit with your doctor, pharmacist or other health care provider. This brings an added benefit - you will become a more active member of your health care team.

Our Pharmacists

Your MTM pharmacist is specially-trained to thoroughly evaluate your medication regimen, to provide education on your medications and medical conditions and to communicate effectively to your other health care providers. Your MTM pharmacist may also be able to administer a variety of vaccines, to provide affordable health screenings and other wellness services, and to offer comprehensive diabetes education services.
